Mine has the game title, the region in parentheses, and "(XenoPhobia)" at the end, before the file extension. None of the ROMs I have for Pokemon games have a number like this in the title. This was entirely foreign to me when I initially read it (still kinda is even after researching it since there's not much info on it, other than 3541 being an older version). The readme for the hack says that the version number must be either 3541, 4997, or 4998, suggesting that it should be in the file name.
I'm attempting to apply a ROM hack to Pokemon Platinum for DS, specifically Renegade Platinum.
